You must first meet with an Acceptance Agent before shipping us your passport application for processing.
After you have completed the DS-11 passport application and have collected all of the supporting documents, both parents and minor must appear in front of an Acceptance Agent to have your passport application executed. The Acceptance Agent will review your documents, ask you to sign the application, they will seal it in an envelope, and hand it back to you. You will ship us this sealed envelope.
Once you have met with the Acceptance Agent, ship all of your required documents to us immediately. We must receive your documents within 4 business days of meeting with the Acceptance Agent.
Note: Many Acceptance Agents may not accept your documents unless they see your departure date is within 14 days. It is recommended to contact them first to check if they enforce this rule.
What does the Acceptance Agent do?

Verifies your identity

Verifies your documents are in order and collects the $35 Execution Fee

Administers an oath in which you will swear that you are submitting true and correct information in support of your passport application. You will sign the application in front of the Acceptance Agent.
You will present the
Acceptance Agent the following documents (unsigned documents will be signed in front of the Agent):

One (1) unsigned Letter of Authorization

Original proof of citizenship

Parents' proof of identity

Statement of Consent: Issuance of a Passport to a Minor DS-3053 (If applicable)

Proof of travel

One (1) passport photo

Check or money order payable to "U.S. Department of State"
I'm ready to apply. How does the Authorization Process work?
You will present your documents to the agent who will verify your identity and review your documents. Let the agent know you will be using an expediting service to "hand carry" your application. If everything is in order you will be asked to sign the application. The agent will then place the application and documents inside an envelope and seal it with time and date sensitive stamps. Make sure the agent returns the sealed envelope to you -
DO NOT open it or the authorization process will be invalidated.
You are now ready to send us the sealed envelope. Please send the sealed, executed envelope together along with an additional copy of the following documents:

One (1) copy of the DS-11

One (1) signed copy of the Letter of Authorization

Order confirmation- this will be available to print after you place your order
Common Passport Application Authorization Problems
Provide a copy of one of the following. (Applicant's name must appear on documentation):

Copy of a plane ticket to a foreign country or;

Copy of travel itinerary indicating dates of international travel or;

Business travelers may submit a [DOCUMENT!]
Note: Most Acceptance Agents will

You will write a check to the "US Department of State" for $181.36 (please write your date of birth on the memo line of the check).
Passport Cards- If you are also applying for a passport card you must send a check in the amount of $196.36 payable to the "U.S. Department of State". Passport cards cannot be expedited and will be mailed to the address indicated on your application.
IMPORTANT NOTICE REGARDING FEES!
The Acceptance Agent may ask you to write a check for a different amount than we have instructed. If so, please contact us immediately to let us know. This is very important as it allows us to ensure the timely processing of your passport application to meet your travel needs.
Starter checks (checks without a pre-printed address) and 3rd party checks are not accepted.
